Vendors are treated like an extension of their own network<\/a><\/li>\n                        <li><a href=\"#topic6\">5. Someone owns security<\/a><\/li>\n                        <li><a href=\"#topic7\">One thing to check this week<\/a><\/li>\n                    <\/ul>\n    <\/div>\n        <div id=\"topic1\" class=\"skip-item\">\n                        <p>Many breaches begin when something predictable happens that a layered strategy could have caught earlier. Here are the five patterns that show up most consistently in organizations with good track records.<\/p>\n    <\/div>\n        <div id=\"topic2\" class=\"skip-item\">\n                        <h2>1. Patching is a recurring calendar event<\/h2>\n                <p>The 2017 Equifax breach exposed the personal information of approximately 147 million people after attackers exploited a known vulnerability that Equifax had been notified to patch. Other control failures contributed to the scale of the breach, but the lesson here is simple: the calendar is one of security&#8217;s most useful tools.<\/p>\n<p>Businesses that reduce their exposure to common attacks treat patching like payroll. Someone owns the process and runs it on a schedule. They watch for urgent security updates and have a plan for coverage when they are on PTO. They don&#8217;t wait for someone to notice something is wrong. This applies to everything: operating systems, applications, and firmware on routers and network devices. The firmware update on your office router is as exciting as a strongly worded memo about reheating fish in the microwave. But it&#8217;s also what closes the door an attacker would otherwise walk through.<\/p>\n<p>A practical cadence starts with risk. Prioritize actively exploited and internet-facing vulnerabilities as quickly as testing allows, then set deadlines for other updates based on their severity and your exposure. Automate what you can. Track what remains, and give someone responsibility for checking it.<\/p>\n    <\/div>\n        <div id=\"topic3\" class=\"skip-item\">\n                        <h2>2. They know who has access to what, and they review it<\/h2>\n                <p>Access creep catches most people off guard. You\u2019re trying to move fast and accommodate the business; doors that were opened in the heat of chasing a hot deal never get closed. A review of who has access to what doesn&#8217;t have to be elaborate, but it does have to happen, and it should be scheduled like patching.<\/p>\n<p>Sometimes access is handed out like free samples at the supermarket, and permissions accumulate like Funko Pops. And just like those wacky big-eyed toys, they collect dust and are forgotten about on a shelf in your spare bedroom. Every user added, every vendor granted access, every service account created for a project that ended eighteen months ago: they grow, and grow. Businesses that manage access well run a quarterly access review by checking critical systems that allow broad access. Who has admin rights? Does it still make sense? Has anyone left the company whose access was never revoked?<\/p>\n<p>A useful starting point for a business that hasn&#8217;t done this recently: pull the admin account lists for your five most important systems. Email, cloud storage, any financial or HR platform, your network equipment, and your primary business application. Count the accounts with elevated access. Check whether every name on those lists should still be there. Most businesses find at least one account that shouldn&#8217;t exist anymore. Even if you find nothing to remove, you&#8217;ve confirmed that the front door is still locked and know exactly who has a key.<\/p>\n    <\/div>\n        <div id=\"topic4\" class=\"skip-item\">\n                        <h2>3. They've practiced before something went wrong<\/h2>\n                <p>Mike Tyson said that everyone has a plan until they get punched in the mouth. An incident response plan that exists only on paper doesn&#8217;t survive first contact with an actual breach. Businesses are better prepared to recover when they have tested their plan before they need it.<\/p>\n<p>A ninety-minute tabletop exercise, once a year, built around \u201cThe file server is encrypted. What do we do first?\u201d can surface gaps that a static review may miss. Who calls whom? What gets isolated? What gets preserved? Who talks to customers? Who contacts legal? None of these are hard decisions to make in advance. All of them are very hard to make well at 2 a.m. with all systems locked up and a ransomware note on the screen.<\/p>\n<p>The tabletop doesn&#8217;t require outside facilitation or a formal security consultant. A clear scenario, the right people in the room, and ninety minutes of honest conversation can surface critical gaps. Common findings include: nobody knows where the cyber insurance policy is, the person who knows the backup recovery process is a single point of failure, and the customer notification decision has no clear owner. These problems are much easier to fix before they become urgent.<\/p>\n<p>We have all practiced fire drills our whole lives. We know where the meet-up point is in the parking lot. Security response is no different.<\/p>\n    <\/div>\n        <div id=\"topic5\" class=\"skip-item\">\n                        <h2>4. Vendors are treated like an extension of their own network<\/h2>\n                <p>The 2013 Target breach exposed approximately 40 million payment card accounts. The attackers used credentials stolen from an HVAC vendor to gain an initial foothold in Target&#8217;s network. Access intended for a contractor became a path into a much larger environment.<\/p>\n<p>Every vendor with access to your systems is a potential entry point. Businesses that take security seriously give vendors the minimum access they need for the specific task, review that access periodically, and remove it when the engagement ends. Someone owns security<\/h2>\n                <p>Yes, we preach that everyone needs to be security-minded, that it is a team approach. That is true, but every team has a captain or a coach. One person, even if security is only part of their role, can be the difference between security getting done and security getting deferred indefinitely.<\/p>\n<p>When something needs patching, an access review is overdue, or a vendor asks for permissions broader than they need, one person notices and another acts. That person doesn&#8217;t require a CISO title or a dedicated security budget. They need ownership and the authority to act without being overruled for convenience.<\/p>\n<p>In smaller organizations, this is often an IT generalist, an office manager with technical aptitude, or the business owner. The role doesn&#8217;t need to be full-time or come with a new title, but it does need to be clearly defined. It needs to exist. Making it explicit and putting someone&#8217;s name to it is the difference between a security program that runs and the concept of a plan from a presentation two years ago.<\/p>\n    <\/div>\n        <div id=\"topic7\" class=\"skip-item\">\n                        <h2>One thing to check this week<\/h2>\n                <p>Pull up your admin access list for your most important systems. Count the accounts with full administrative rights.
